Food Allergy Information
The information asked in each question is required to ensure your safety, health, and welfare while studying in TU Dublin and to ensure that appropriate assistance is provided to reasonably accommodate personal safety.
The Safety, Health & Welfare Team and the relevant School will receive the information disclosed. All information provided will be treated as strictly confidential and used only to ensure your safety. You may be approached in confidence by the Safety, Health & Welfare Team/Head of School/Assistant Head of School to ensure if necessary a risk assessment and a first-aid response plan is completed.
You shall not be permitted to complete the registration process and access kitchens/laboratories/work areas until the questionnaire is completed.
It is your responsibility to ensure for your own safety while studying in TU Dublin that you disclose this information so that appropriate assistance is provided to reasonably accommodate your personal safety.
If you have an allergy you will not be permitted to access the university's kitchens/laboratories/work areas until the risks associated with that allergy are assessed.
You must provide supporting medical documentation to the Safety, Health & Welfare Team. Supporting medical documentation must include the severity of your food allergy, your fitness to be in contact with the food allergen(s), any actions/controls recommended to ensure your safety while working with said allergen(s).
